Diabetes Tasmania

The National Diabetes Services Scheme (NDSS) is an initiative of the Australian Government administered with the assistance of Diabetes Australia. In Tasmania the NDSS is administered through Diabetes Tasmania. The NDSS delivers diabetes-related products at subsidised prices through community pharmacies and provides information and support services to people with diabetes. Registration on the NDSS is free and open to all Australians diagnosed with diabetes. The Tasmanian Health Services, Health Promotion Coordinator Jacquie Maginnis is responsible for the Arts and Health Project. She will provide information about evidence, guidelines, projects and networking opportunities that support the “Creative Ageing Movement”.

Arts in health promotion is founded on a growing body of research which affirms the importance of arts for our health and wellbeing. Creative ageing is a protective factor contributing to the social inclusion of older adults, and combating ageism and the fear of ageing. Corumbene Care is  a well-established not for profit provider of health and aged care services in the Derwent Valley, and also delivers services in the local government areas of Southern Midlands and the Central Highlands. Corumbene Care has an excellent reputation for high quality care, in both residential and home care services, and more recently in rural primary health care.

Recently Corumbene Care developed an innovative rural primary health program for clients living with chronic diseases, through improved care coordination and support, combined with telehealth strategies. The telehealth component, focused on remote vital signs monitoring, has been implemented with support from Tunstall Health Care – Corumbene’s project partner. Corumbene Care acknowledges the support by Primary Health Tasmania under the Australian Government’s Primary Health Networks program for this service.
